I/We have not received nor been induced by any rebate or gifts, directly or indirectly, in making this investment. Restriction on Acceptance of Third Party Payments for Subscription of Units Nippon India Mutual Fund has vide its Addendum No. 56 dated November 12, 2011, has provided the details pertaining to restrictions of Third Party Payments with effect from November 15, 2010. Therefore, for payments through net banking and debit cards, NAM India shall endeavour to obtain the details of the bank account debited from the payment gateway service provider and match the same with the registered pay-in accounts. In case it is found that the payment is not made from a registered bank account or from an account not belonging to the first named unit holder, NAM India /RTA shall reject the transaction with due intimation to the investor. Currently, where the investor account details are not made available by the payment gateway service provider, NAM India shall obtain the name of the bank making the payment for the subscription. I hereby authorize the representatives of Nippon Life India Asset Management Limited. and its Associates to contact me through any mode of communication. This will override registry on DND / DNDC , as the case may be.

By signing this SIP enrolment form I/We understand that the amount will be debited from the Bank account mentioned in One Time Bank Mandate / Invest Easy - Individuals Mandate Form. I hereby authorize the representatives of Nippon Life India Asset Management Limited its Associates to contact me through any mode of communication.
